What are the requirements and how to apply for Social Security disability benefits?
In order to claim Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI), a person with a disability must share certain personal information, information about their medical condition, information about their job. You must submit the following documents :
- Original birth certificate, certificate, or other proof of birth
- Proof of citizenship or immigration status
- United States military service papers performed before 1968
- Copies of W-2 form and/or self-employment tax returns from last year
- Medical evidence of disability (medical records, doctors’ reports, and latest test results)
- Approval letters, pay stubs or payroll stubs, settlement agreements, or other evidence ofsome other type of temporary or permanent benefit
How to apply for SSI or SSDI?
Adults with a disability can apply for SSI online. People applying for a child under 18 with a disability or a person over 65 without a disability should visit their local Social Security office or call 1-800-772-1213 between 7:00 a.m. and 5:00 p.m. 7:00 p.m., Monday through Friday.
